Online Student Nominee Form for an
IEEE Canada Power Quality Scholarship
sponsored the Trustees of the Alberta Power Quality Conference

Read the entire form before you start.   It is recommended that you create all your content in a text editor, then cut and paste into the form.   Complete all sections of this form and press the submit button.   Only IEEE Student Members in good standing at Universities in Alberta are eligible to receive this scholarship.   This form is to be completed and submitted by the nomineee (student) in the same department at the same university as the nominator (faculty member), and must be accompanied by a second form completed and submitted by that faculty member.   The nominator should automatically receive a copy of the output of this form by email.   You will not be able to submit this form unless you complete ALL sections.


Step 1:   Please identify yourself as the Nominee

1.1   Enter your full name - surname last

and your IEEE membership number enter 0 if you are not a member

1.2   Select your university from this list:
(if your university is not on this list, it means you are not eligible for this Scholarship)

1.3   Enter the name of your academic program     (e.g. electrical engineering)

Step 2:   Please identify your Nominator

2.1   Enter your nominator's full name - surname last

your nominator's e-mail address

Step 3: Please provide details of your nominees interests and acomplishments related to this scholarship

3.1   Please list the courses that you have taken (or intend to take) in electric power subjects
        (e.g. power circuits, electric machines, power systems, power electronics, etc.)
        and provide examples of your involvement that demonstrate your interst in this subject
        (up to 600 characters)

3.2   Explain what you understand to be implied by the term "Power Quality"
        (up to 300 characters)

3.3   State/explain your view of the three most important issues in Power Quality
        (up to 300 characters)

3.4   Describe your interest in Power Quality
        (up to 300 characters)

3.5   Describe your goals as a future highly trained professional
        (up to 300 characters)

3.6   Explain what will this scholarship mean to you if your application is sucessful
        (up to 300 characters)
